These forms are available to our clients (past participants of the Wilderness Leadership Training) to download and print out. To view these documents you will need Microsoft Excel or a recent version of Adobe Acrobat Reader.

Please fill out the appropriate spreadsheet and email it to the Oakland program team (gear at bawt.org) or email it to the South Bay program team (gearsb at bawt.org). In the subject line please put Oakland or Milpitas Gear Request Form, your name, and the date you want to pick up the gear. Please give accurate equipment numbers and make sure to include pick up and drop off dates and times.

Gear Library Blackout Dates: The gear libraries will be closed around the WLT dates. We will be closed in May 2010. Please take note and plan accordingly.

>Trip Report Form
The Trip Report Form should be turned in when equipment is returned. This form is critical as it is used to help report numbers of youth served to our funders.

>Photo and Testimonial Contest
After your trip, send BAWT your photos and testimonials – you could win a $25 REI gift certificate.

>Workshop Registration Form (PDF document)
The Workshop Registration Form is designed to create ease in registration for BAWT courses. After you have spoken with BAWT staff to ensure that there is space available in the given course or workshop, you may print a copy of this form and send it to BAWT with your payment.
